For mild community-acquired pneumonia in adults, oral amoxicillin 1g three times daily for 5 days is the recommended first-line treatment per most guidelines including NICE and Therapeutic Guidelines. For moderate-to-severe CAP requiring hospitalisation, intravenous benzylpenicillin plus oral doxycycline is standard. A macrolide such as clarithromycin should be added if atypical pathogens are suspected or the patient is not responding.
